We are looking for an enthusiastic Registered Nurse Associate (RNA) to join our innovative Virtual Ward Team as a Discharge Clinician. As part of the Virtual Ward Team, you will play a vital role in ensuring smooth and safe discharge from acute hospitals for patients, supporting their transition to ongoing home-based treatment.

Main duties of the job

This role is an opportunity for a Registered Nurse Associate with NMC registration and an appropriate Foundation Degree.

  • Assess and prepare patients for discharge from the acute hospital onto the Virtual Ward (VW), ensuring they are ready for a safe transition to community care at the place they call home.
  • Collaborate with multidisciplinary teams, including doctors, nurses, and allied health professionals, to identify patients suitable for the Virtual Ward, identifying clinical needs and liaising with the VW Hub coordinators.
  • Be able to consider a range of factors regarding service capacity/occupancy and consider this when reviewing potential patients to meet clinical priorities/consider acute flow pressures/VW availability.
  • Work together with the VW Hub to complete onboarding processes and handover care and escalation plans.
  • Coordinate necessary equipment, medications, and any care/AHP support to facilitate timely and efficient discharges onto the Virtual Ward caseload.
  • Provide education and support to patients and their families on the VW service.
  • Ensure accurate documentation of patient records, discharge plans, and communications with healthcare professionals.

Person Specification

Qualifications

Essential

  • Fd in Health & Social Care.
  • NMC Registration as a Nursing Associate.
  • Fully Completed Care Certificate.
  • GCSE Grade A-C English & Maths or Equivalent.
  • Evidence of continuing professional development.

Desirable

  • Have completed training or study in areas relevant to the role applied for, or be willing to undertake, for example: venepuncture, urethral catheterisation, leg ulcer dressings (depending on area of practice).

 

Experience

Essential

  • Experience relevant to the area of practice applied for.
  • Experience of working with patients & families from a range of cultures & backgrounds.
  • Experience of self-directed working and working as part of a team.
  • Evidence of good written and verbal communication skills.

Desirable

  • Have experience in more than one area of practice.
  • Other health care-related experience.
  • Community or Social Care experience.
