Are you a qualified social worker with a passion for supporting individuals with mental health needs? Do you have expertise in care coordination, safeguarding, and mental health legislation? If so, we have an exciting opportunity for you to join our Adult Mental Health Team (AMHT) as a Social Care Specialist.

Role Overview:

As a key member of the AMHT, you will bring your social care expertise to the team, working alongside multidisciplinary professionals to ensure that individuals receiving support have their full care and support needs assessed under the Care Act 2014. You’ll play a crucial role in coordinating support plans that address these needs, empowering service users and their families to make meaningful decisions about care.

Main duties of the job

1. To work independently as part of a multi-disciplinary team to: assess, plan and implement patient-specific programmes of care/interventions within the Care Programme Approach, contributing to the continuous holistic assessment of patients.

2. To participate in and contribute to the multi-disciplinary team, articulating the social care perspective and ensuring that meeting the social care needs of service users retains a high priority and profile within the functioning of the team.

3. To undertake Care Act assessments some of which will be for patients presenting with highly complex needs using your profession-specific knowledge and skills.

4. Ensure that there are no delays to service users receiving appropriate care and support by completing the relevant process to access care funding and implementation of support plans.

5. To function effectively across a variety of environments (e.g. AMHT, ward, department, service users homes etc.) involving changing and demanding conditions.

 

Person Specification

Occupational Experience

Essential

  • To have appropriate, relevant experience which should include a thorough understanding of working within community mental health services.
  • To understand and have experience in Risk Management.
  • To understand the legal responsibilities of the profession.

Desirable

  • To have experience in supervising student placements.
  • To have experience in supervising other staff.

 

Other requirements

Essential

  • To comprehend and work within the Trust policies of data protection, equal opportunities, Health & Safety and to meet the diverse needs of patients
  • Able to travel to undertake work and have the capacity to carry equipment relative to your post
  • To be able to manage in a stressful working environment and with emotional or aggressive patients/or carers

Desirable

  • Car driver with car available for work.

 

Education and Training

Essential

  • Professional Qualification (Degree or equivalent).
  • Professional Registration.
  • Evidence of Continuing Professional Development (CPD) portfolio.

Desirable

  • Post graduate education specifically relating to client group (Post basic qualification in community care/ research awareness/ management & leadership).
  • AMHP Qualification.
  • BIA Qualification.
